In the first place, the xy vs. xx definition has been mostly discounted in recent years, and does not serve as the basic determination of gender anymore.
And right now, the picture suggested that she has not yet undergone treatment, meaning that she technically still has male genitalia, even though the rest of her body and hormone balance fits that of a female. Legally, then, she is not a female, but will be in some time.
Plus, the MOST important determination of gender is the mind, and the role one wishes to take in society, both of which are, in her case, decidedly female.
Once she has undergone surgery, she will be female, and even now she deserves the female pronoun.

If you're a boy and act significantly girlish, you're transgender.

At both, they don't and can't. Post-op male-to-female transsexuals might as well be post-menopause. They don't menstruate, they can't get pregnant, and their bodies can't produce estrogen by themselves.

Wanting a sex change isn't that related to having sex. Somebody like me who is male, wants to be female, and is attracted to females doesn't just have a lesbian fetish. If I had to stay a virgin my whole life, I'd still want to be a female rather than male just as much. Also, just because I'm attracted to females and want to be a female, that doesn't mean I'd want be aroused by my own body. It's also much easier for either completely straight males or lesbian females to find a good girlfriend than transsexuals.

The process by which gender is determined is not very well understood, but both mental gender identity and your physical sex are determined by hormones given in the womb. Supposedly, a mind treated with female hormones will want to act in a generally accepted "female role" (however this is defined in the current society).
I find this explanation to be a little too absolute, because there seems to be a gradient for both mental gender and sexuality, but it's approximate and explains that transsexuality is biologically based..

The US has restrictions on the legal age to begin hormone treatment, so you can only start at 18. It's more lax in Europe and Canada, which is why this kid is so lucky. She hasn't even gone through puberty yet.

In taking artificial hormones, [male-to-female] transsexuals take both antiandrogen (to cancel the effects of male hormones) and estrogen (to give female hormones). The former prevents male puberty from happening, and the latter triggers something similar to a female puberty.
Not that a male puberty can cause someone to grow a dick, but she doesn't even experience male puberty (probably was just starting it before taking hormones). Saying she's already 16 and started taking the hormones at 12, she's already pretty much done with most of that puberty.
