This happened when you were trying to ignore an actual user, right?

I would guess that to SMF "guest" is the null user and by extension the result when it tries looking up a user and fails for whatever reason. So, instead of adding the legitimate and intended user name to your ignore list, it added "guest". If so, this would be just a typical glitch that doesn't happen often and isn't worth trying to fix.
