I use KeePass and generate random passwords for everything except a few things where I have secure passwords I have memorized.

I used to use a small set of passwords

Half a year ago, I switched to password management software, and chose password-store, which is some simple commandline thing that stores them with GPG encryption, generates with pwgen, and allows entering plaintext detailed info for each entry alongside a password.

i have a general pattern of letters and numbers i use for almost every password. there are variances between them but they're all very similar. it's not a word, but rather a pattern on the keyboard. so it looks like gibberish.

I use one or two NATO call signs and alternate between ending in 01 or 02. In fact all my passwords are military related.
