We don't have a valid permission form on file for your son at this time.   If
there was a problem with the e-mail address on the form, you'll need re-submit
the form with the correct, valid e-mail address.  Once we receive and process
the completed form, we can reactivate the account.

If there's room on the form, you can make corrections on it and send it back in,
or print out a new form and start from scratch.  The My Nintendo parent/guardian
permission form is available online at the following URL:

http://www.nintendo.com/parents

The link to the form is on the left-hand side of the page.  This page also
provides information that may address any questions your parents may have
regarding the My Nintendo program.

If you have any questions about the My Nintendo program that are not addressed
at our web site, or if you are unable to print out the permission form, please
send an e-mail to registration@noa.nintendo.com or call Nintendo's Online
Privacy Policy Administration at 1-888-977-7627.  Representatives are available
8 a.m. to 5 p.m. Pacific time, Monday through Friday.
