It looks like I can rule all of those out except Escape from Monster Manor pretty easily, though. It was definitely a Wolfensteinesque shooter, but it was most likely on DOS (not 3DO like Escape from Monster Manor) and it had multiple weapons (also not like Escape from Monster Manor).

The Dark Eye looks cool as hell though.

This definitely seems to be the closest match I've seen yet, although very colorful compared to my memory. This opening level feels very familiar, and I do remember floating eyeball things, either as enemies or as part of the scenery.
