I'm really lazy and I save almost everything into the default "Documents" folder on OS X, but I generally take a day or two out of every month to sit and organize everything and delete unwanted stuff.
Though if it's a file I want to use right away and then delete afterward, like an image to crop into an avatar or something, I'll save it to the desktop, use it, then delete it right away. My desktop rarely has more than a couple files on it. So at least that part is organized usually.

I usually just save stuff in "Downloads", but I go through it every once in a while and most of my stuff's somewhat organized.
I was actually considering making a thread about file organization just a couple days ago, but decided not to because it probably would seem too obsessive. girl;
and yes, I totally organize my computer. I have a main storage drive containing folders such as: code, Documents, Downloads, Installers, ISO, Music, Pictures, ROM, Torrent, and Videos. I keep my Downloads folder relatively empty, my Torrent folder only keeps things while they're still on a seeding queue, and my operating systems are set to use some of these instead of the regular locations for "My Documents" and such.
I'm super super anal about how organized my files are. I start to freak out if I see a file or folder that I don't reconize.

I am like this too. I have like folders within folders for specific random things
generally not so much... I only save things I will use over again like ROMs into specific folders
Eh fairly organized. My desktop is usually clutterfree, but then I'll have some sort of project that requires a lot of files that end up getting spread over my desktop. As long as I can access a file within a few clicks and minimal searching time, I don't really care. I probably clean things up every couple months or so.
I usually only make folders if there's going to be a lot of stuff related to whatever it is. So I sort of have it organized but it's still not completely organized.
Do I bother to keep the files orderly in a GUI? No. I thankfully never have to defrag my Mac as it isn't prone to fragmentation as a PC.
My pc and my macbook are a god damn mess in terms of file organization.
If I want to use something immediately I save it to the desktop because searching for it is just so much work n_u
And my downloads folder is never really maintained, there is so much shit in there. Really I just save things wherever I feel like saving them at the time...which is never the My Documents folder.
My computer is more organised then my room.
