Currently they are only available on Adium, Pidgin, and Digsby.
If your IM client supports emoticon sets and there isn't currently one, just post an example emoticon set and the name of your client and I'll try to get one up.

In a chat window go to all emotes and there should be an Add option.
They can be fairly large i believe

I edited the set as of this post to make it be identical to the Adium set, by removing the two I listed above.

On the buddy list, click tools->preferences
click the smileys tab
drag-and-drop the file from windows explorer into that prompt (or click the prompt's add and navigate to it)

By the looks of it, I don't think you can add your own smileys.
