It is a practical impossibility for any current nation and or organization to launch enough ICBM/SLBM's at the CONUS to "kill everyone". No one has that kind of capability or the means to develop it. There are enough defenses in place, both passive i.e. shelter, sheer number os people/land mass, and direct i.e. military countermeasures to prevent such. In any case, any ICBM/SLBM can hit any point on Earth within 30 minutes, not one or two hours, unless they are cruise missiles; but they don't have the range or capability required to do the above described deed.

If any nation were to attack the US they would also likely attack Canada due to our mutual defense pacts, common interests, mutual military operations etc... So you might get directly killed there just the same. If not the radioactive fall out would be immense on the North American continent and affect millions who survived the initial blasts. It would last for years, if not decades, so it would not be a crisis, more like an epic disaster.
